Belumosudil is an orally active, selective Rho-associated coiled-coil containing protein kinase 2 (ROCK2) inhibitor used in the treatment of chronic graft-versus-host disease (cGVHD). It modulates inflammatory and fibrotic signaling pathways by inhibiting ROCK2, which regulates T-cell activation, cytokine production, and tissue fibrosis. Belumosudil has shown efficacy in patients with cGVHD who are unresponsive to multiple prior lines of therapy.
It is approved under the brand name Rezurock® in the United States.
Therapeutic indications include:
• Chronic graft-versus-host disease (cGVHD) after failure of at least two prior systemic therapies
• Investigational use in fibrotic and autoimmune diseases

Impurities in Belumosudil arise from its multi-step small molecule synthesis, which involves heterocyclic ring formation, halogenation, amide coupling, and selective substitutions. Due to its complexity and long synthesis, impurities—especially genotoxic and structural isomers—require careful control in accordance with ICH Q3A/B and ICH M7 guidelines.
Process-Related Impurities
• Unreacted intermediates such as substituted pyrimidines or aminopyridines
• By-products from acylation, halogenation, or ring formation steps
• Residual reagents like EDC, HOBt, or coupling agents
• Residual solvents such as DCM, methanol, or DMF
• Isomeric or positional analogs from uncontrolled substitution patterns
Degradation Impurities
• Hydrolysis of the amide bond under acidic or basic conditions
• Oxidation of the aromatic or heterocyclic rings
• Light- or heat-induced rearrangement or degradation
• Formation of deschloro or hydroxylated derivatives
Elemental Impurities
• Trace metals such as palladium, copper, or iron from catalytic or synthetic steps
• Controlled under ICH Q3D limits for oral drug products
Analytical Techniques for Impurity Detection
Belumosudil is used for the treatment of chronic graft-versus-host disease (cGVHD) in patients who have failed at least two previous systemic treatments.
It inhibits ROCK2, a kinase involved in immune regulation and fibrosis, thereby reducing inflammation, fibrotic tissue remodeling, and cytokine production.
Yes. It is a targeted small molecule inhibitor specifically designed to modulate ROCK2 signaling pathways.
Store at room temperature (20–25°C), protected from moisture and light. Keep in the original container until use.
