Alogliptin, sold under the brand names Nesina and Vipidia, is an oral anti-diabetic drug in the DPP-4 inhibitor (gliptin) class. Like other members of the gliptin class, it causes little or no weight gain, exhibits relatively little risk of hypoglycemia, and has relatively modest glucose-lowering activity. It is used to improve glycemic control in adults with type 2 diabetes and is often prescribed as part of a comprehensive treatment plan that includes diet and exercise. Alogliptin may be used alone or in combination with other antidiabetic medications.

During the production and storage of Alogliptin, impurities may form, which must be controlled to ensure the drug’s safety and efficacy. Regulatory authorities, such as the U.S. Food and Drug Administration (FDA) and the European Medicines Agency (EMA), set guidelines on permissible impurity levels in Alogliptin formulations.
Alogliptin is not recommended for individuals with type 1 diabetes, diabetic ketoacidosis, or a history of severe allergic reactions to the drug. Patients with significant liver or kidney impairment should also avoid or use it with caution under strict medical supervision.
Common side effects of Alogliptin include headaches, upper respiratory tract infections, and gastrointestinal symptoms such as nausea or diarrhea. Rare but serious side effects include pancreatitis and severe allergic reactions like Stevens-Johnson syndrome.
Alogliptin is usually taken once daily, with or without food. While the time of day is not critical, it is best to take it at the same time each day to maintain consistent blood levels and improve adherence to treatment.
Patients should be cautious of symptoms of pancreatitis, such as severe abdominal pain, and report them immediately to their healthcare provider. Regular monitoring of kidney and liver function is advised, especially in patients with preexisting conditions.
